• Request Medical Records from St. Anthony Hospital

  • Obtaining your medical record.

    Access to medical records is available to patients over the age of 18 or a legal guardian. Records are available in two ways:

    Free Online Electronic Health Record

    Free online access to most records through MyCenturaHealth

    Create an online patient account for a safe and simple way to access information from your electronic health record (EHR). For privacy and security reasons, some records will not be available online. Get started here.

    Hard Copy of Medical Records

    Fees for Medical Record Copies:
    Fees for copies of medical records for a patient/personal representative as pursuant to Hi Tech Omnibus Rule HIPAA 164.5249(c)(4), effective 9/23/13, are:

    • 14 cents per page for electronically stored records, plus postage.
    • 16 cents per page for paper based records, plus postage.

    To request records by mail:

    • Download and complete the Patient Request to Access Medical Records Form or en Español

    • An invoice from our business associate, MRO will precede the actual copies and the copies will be sent once payment is received.
    • You can pay your invoice by check or credit card.

    To pick up records in person:

    • Download and complete the Patient Request to Access Medical Records Form or en EspañolUnder your address information on the form please write "will pick up in person."  Please fax to 720-321-3341 or mail it to:

    • Once your records are ready we will call you and you can pick them up, Monday through Friday, 8 a.m. to 4 p.m.  in the Medical Records Department. Payment may be made by credit card, check or cash. Credit card or check is preferred.

    If you need your records sent to another provider you will need to submit authorize the disclosure of protected health information to third parties.
